Information on Nursing Home Transition and Diversion

Program Description

Nursing Home Transition and Diversion (NHTD)is a special program funded by the New York State Division of Housing & Community Renewal (DHCR) and the NYS Department of Health [DOH]. NHTD provides clients alternatives to nursing home placement and allows clients the opportunity to choose housing in areas where they feel safe and comfortable. The Rochester Housing Authority provides a rental subsidy to landlords on behalf of the client. Participants will pay 30% of their annual income towards rent and utilities.

Who is eligible for the program?

Potential clients must be people with disabilities ages eighteen (18) or over or seniors.

How do I apply?

Participants who wish to enroll in NHTD must complete the eligibility process with a Regional Resource Development Center [RRDC].
