ELDERSOURCE SERVICE COORDINATORS

Rochester Housing Authority has contracted with Eldersource to provide social work assistance to our residents who live in our designated elderly or disabled housing. Eldersource is a collaborative effort between Catholic Family Center and Lifespan.

The Eldersource Service Coordinators are trained professionals who are available to assist our residents in a variety of ways.

The primary goal of the Eldersource Service Coordinator is to improve or maintain the quality of life of Housing Authority residents by linking them to available community services or resources.

Service Coordinators assist RHA residents in many ways such as:

• Reviewing and obtaining all available/appropriate benefits
• Assisting tenants with paperwork as needed
• Providing emotional support/counseling
• Assisting tenants with legal issues
• Providing medical linkages
• Assisting with necessary home services
• Assisting with yearly HUD recertification
• Helping tenants to understand their responsibilities as HUD tenants
• Advocating for tenants with building management when necessary


Service Coordinators possess a wide range of knowledge and materials encompassing benefits, health care, insurances, in-home services, human service resources, socialization, educational and training opportunities. Their services are free to RHA residents and confidential.
